Athletics Registration

Athletic participation at CCHS requires that all students be registered on FamilyID by a parent or guardian and have a valid physical on file for each season. FamilyID is an online platform that allows the athletic office to maintain rosters and health records and credit families for user fee payments. No student is allowed to participate in a tryout, practice or game without a completed registration.

Registration

Visit FamilyID and please complete all information requested. FamilyID only requires parents to fill out all sections the first time you register. During every successive season the information will pre-populate. FamilyID customer support is available weekdays from 8am – 9pm and weekends from 9am – 8pm at 800-311-4060.

User Fees

The CCHS user fee for all sports is $300 per season, per child. The family cap is $900 per school year. If you have reached the family cap, please be sure to check that box during FamilyID registration. Families can pay via PayPal on FamilyID or can send a check payable to CCRSD to Aaron Joncas in the Athletic Office.

Physical Exams 

State law requires students to have a physical on file with the school nurse that explicitly notes that the child is medically cleared to participate in all school activities. Physicals are valid for 13 months from the date of the exam and must be completed in-person. No student is allowed to participate in a tryout, practice or game without a valid physical on file. There are no exceptions to this rule. FamilyID will automatically notify families at least 30 days in advance via email when a physical is set to expire.

Student physicals can be dropped off or mailed to the CCHS Athletic Office, faxed to the Health Office at 978.318.1413 or emailed to healthservices@concordcarlisle.org. Transfer Students

Students entering CCHS in grades 10-12 or as a repeat 9th grade student are required to seek an eligibility waiver from the MIAA, according to Rule 57 of the MIAA Handbook. Please contact Athletic Director Aaron Joncas upon enrolling at CCHS to initiate the process. Please feel free to contact the Athletic Office at any time with questions regarding this rule.
