The Massachusetts Department of Public Health requires that all school-aged children have periodic physical examinations. Physical examinations forms must be on file in the Health Office at CCHS. The only exception is for documented religious reasons. Physical examination requirements for high school students are as follows:
- New Students: Within the year prior to starting school. They must also have completed a Health Screening Form.
- Sophomores: Before the school years begins, or 1 year from the freshman physical exam.
- Athletes: Annually, with current physical on file prior to practices or try-outs. This form must state whether or not the student is physically fit for competitive sports and whether there are any restrictions.
Parents or guardians are responsible for arranging physicals and forwarding the forms to the Health Office.
