Fall Sport Information
Athletic participation at CCHS requires that all students be registered on Family ID and have a valid physical on file.
Students can register on Family ID (http://www.familyid.com) beginning 6/1/2018.
Physical Exams are only valid for 13 months from the date of the exam. Student physicals can be dropped off at the Athletic Office, mailed to the Athletic Office, 500 Walden Street, Concord, MA 01742, faxed to the Health Office 978.318.1413 or emailed to email@example.com.

Physical Exams are now due to State Law only valid for 13 months from the date of the exam. Students who have a valid PE at the start of the season and the expiration date occurs during the season becomes ineligible to participate on the date of expiration. The state law requires that student/athletes along with their parent(s)/guardian(s) complete the concussion course. During registration you will be asked to sign off that you have completed one of these courses http://www.nfhslearn.com/electiveDetail.aspx?courseID=15000 or http://www.cdc.gov/Concussion. ImPact testing occurs at the beginning of each school year for 9th graders. If your child has not had the ImPact test and you would like them to please have your child schedule an appointment with either the athletic director or the school nurse. To check if registration has been completed, go to http://www.familyid.com and log in. All registrations are listed on your ‘dashboard’.
If you do not see a completed registration for winter sports, your child is NOT registered. If you have an ‘Incomplete Registration’, click on the button to ‘edit or confirm’ to complete the registration. If you have a completed registration, click on ‘view’ to see the registration details. If you have any questions about whether your child is registered for fall sports, log into your FamilyID account and check your registrations before the first day of tryouts to make sure your registration is complete.FamilyID customers support is available every weekday from 8 am – 9pm EST and weekends from 9 am – 8 pm EST, and we return all messages promptly. FamilyID will be closed on Thanksgiving.
