President's Volunteer Service Award Application

The Young Adult and Teen President's Volunteer Service Awards are given by the President of the United States to individuals who have demonstrated outstanding volunteer service and civic participation over the course of a 12 month period. The 2Volunteer Committee at CCHS is a designated certifying organization.

This award is a self-nominating process. Questions about the Award should be directed to 2Volunteer by the student. All service must be documented and verifiable. The student must fill out the reflection portion of this application.

Please note that applicants are responsible for monitoring their CCHS email accounts for any notification and/or questions from 2Volunteer about their applications.

If you select the Teen Application you must be under the age of 16 at the time you performed ALL of your service hours. The minimum number of service hours to qualify for a Teen Award is 50. If you select the Young Adult Application, you must have a minimum of 100 hours to receive a Young Adult award.

For a Teen award the minimum number of hours is 50. For a Young Adult award the minimum number of hours is 100.
